We are currently determining if additional events will be available in Spring 2026.
Enrollment on Personify Health allows for electronic signature verification via email. Please note that it can take several weeks for an enrolled individual to appear in the screening eligibility file. For that reason, we are encouraging enrollment in Summer/early Fall before online scheduling is available.

Please note that Employees and Spouses/Partners must utilize the appropriate link below to authenticate online signatures before being permitted to schedule.
For the sake of this program, please make sure you know which classification you fall under:
Employee: This is the 51ยายื employee who pays for the benefits out of their pay-slip.
Spouse/Partner: This is the person who is on University benefits, but the cost of those benefits comes out of the pay slip of their spouse or partner. Even if you are a University employee, you are considered a "Spouse/Partner" if you are not the one paying for the benefits.
If you are a new employee or recently enrolled in the University's medical plan and are having trouble with Step Two, please email HealthyOHIO@ohio.edu so we can assist you.
If you recently enrolled in Personify Health and are having trouble with Step Two, please email HealthyOHIO@ohio.edu. Provide the name and email address associated with your Personify account.
A Consent Form must be signed to be able to attend an appointment. Please do not schedule for your spouse/partner without their Consent Form on file. Appointments scheduled without a Consent Form on file will be cancelled.
Walk-ins will not be accepted.
If you select your preferred date for a screening and no appointment times appear, that means that the event you selected is completely booked. It is possible that someone might cancel, but we recommend confirming an appointment for an alternate date to ensure you get an appointment.
If you are unable to make your scheduled appointment, please cancel your appointment so it becomes available for others. To cancel an appointment, please refer to the email you received when you originally scheduled the appointment. Use the link in that email to cancel and, if you wish, reschedule your appointment.
It is recommended that you fast 8-12 hours for your screening. However, if you need to schedule a time when fasting is difficult, don't fast. Inform the clinical team you are non-fasting and a non-fasting parameter for results will be used. Water is always allowed and is encouraged.
